| Appointments Office hours are Monday through Friday, 8:00am - 5:00pm. Call the Yale office at (918) 494-9300 or SouthCrest office at (918) 294-0105 to schedule an appointment. We welcome your calls. Should you have an non-emergency question that our staff is unable to answer immediately, you will be contacted with the answer by the end of the business day. Plan to arrive early, at least 15-30 minutes, to register at the reception desk and/or change into comfortable exercise clothes for physical therapy. Please notify us if you will be delayed. If you are a new patient at the center, you will be registered and asked to review and sign a consent and release form and your insurance information will be reviewed. At the time of initial registration, you will be provided a several page patient history questionnaire. This is to be completed as accurately as possible to provide information to the multi-discipline treatment team. This information is used to provide the best possible treatments to meet your needs and for future analysis of your treatment’s effectiveness and efficiency. Precertification will be obtained as necessary by our precertification specialist. For your convenience, our patient forms are available for download. If you prefer, you may complete the form and bring it with you at the time of your appointment. You will need Adobe Acrobat Reader to view and print the forms. If you do not have a copy of Adobe Reader software, you can download it for FREE by clicking on the icon below. Important: Please note if you are currently using version 3.0 or 4.0 of Adobe Reader, it is strongly suggested that you upgrade to version 5.0 to view these files, otherwise you may experience errors. We have one standard of care for all - the best. Your health comes first. However, high quality medical care for complex problems can sometimes result in bills that may be difficult for you to handle. If this is a problem, please discuss it with our business office staff. Workers' compensation and personal injury insurance are verified by our insurance team and processed for reimbursement. We participate in most local HMO and PPO programs. However, we suggest you verify your eligibility for services with your HMO or PPO insurance. Your personal physician may contact our reception staff to schedule your first appointment. If you choose to call yourself, please be sure to have your insurance information ready and your doctor’s prescription (if for physical therapy). A Reminder At your first appointment, allow yourself an additional 15-30 minutes to register at the reception desk and complete any necessary paperwork. For your convenience, a list of hotels and motels near Eastern Oklahoma Orthopedic Center is available from our reception personnel. At our Yale office, a salad bar/snack bar is available on the ground floor of the Kelly Building, which is located next door, just northeast of the William Building. At our SouthCrest office, a cafeteria/snack area is located in the SouthCrest Hospital adjacent. Eastern Oklahoma Orthopedic Center supports the need for a healthful and safe environment. Therefore, smoking by patients, visitors and staff is prohibited on all William Building property including the Eastern Oklahoma Center, as well as, the Physical Therapy Center. |
