The Registration Process Plan to arrive early, at least 15-30 minutes, to register at the reception desk and/or change into comfortable exercise clothes for physical therapy. Please notify us if you will be delayed. If you are a new patient at the center, you will be registered and asked to review and sign a consent and release form and your insurance information will be reviewed. At the time of initial registration, you will be provided a several page patient history questionnaire. This is to be completed as accurately as possible to provide information to the multi-discipline treatment team. This information is used to provide the best possible treatments to meet your needs and for future analysis of your treatment’s effectiveness and efficiency. Precertification will be obtained as necessary by our precertification specialist.
