Scheduling

Your personal physician may contact our reception staff to schedule your first appointment. If you choose to call yourself, please be sure to have your insurance information ready and your doctor's prescription (if for physical therapy).

A Reminder

At your first appointment, allow yourself an additional 15-30 minutes to register at the reception desk and complete any necessary paperwork. The forms you will need to fill out are available for download on this website.
